The 2001 OHS Annual Survey of work injuries and illnesses involving semiconductor workers is now underway. This Survey is open to all semiconductor manufacturing companies and is sponsored by the Semiconductor Industry Association (SIA). There are no costs associated with participation in the Annual Survey. Each participating company receives a set of "Benchmark" data tables and graphics which compare the company's work injury and illness experience with that of the industry as a whole. The cut-off for data submission in March 1, 2001. The preliminary data presentation will be made at the meeting of the SIA Environmental & Occupational Safety & Heath Committees in Washington, DC, on March 13, 2002. The 1999-2000 Survies included approximately 80% of the US semiconductor industry by employment.

Data Submission Procedure
LEVEL 4 - OHS Software Users (ver. 2.8)
Use the BACKUP option on the UTILITY menu to backup data to a floppy diskette. E-mail the following files to the OHS e-mail address listed below:
CASE.DBF CASE.CDX CASE.FPT
Please Include the total hours worked for each facility during 2001. Alternately, simply mail the backup floppy diskette to the OHS address listed below.
LEVEL 3 - Non-OHS Software Computer Files
Submit non-OHS computer data file(s) in any format, together with a description of the data file. Include total hours worked for each facility during 2001. [Be certain that case characteristic codes match the OHS codes. [If uncertain, contact Don Lassiter to discuss the OHS code re-mapping service.]
LEVEL 2 - Hard Copy Data (OSHA-200 Log)

All data and information submitted remain the property of the participating company and will not be used to identify any company by name or location. Only incidence rates and percentages based on the data submitted with be presented in the 2000 Survey Report. No employment data or numbers of cases, lost workdays etc. will be presented, except in compiled format to prevent identification of participating company data.
